چکیده

Introduction Abnormalities of horizontal gaze are common in strokes involving the caudal part of the pons and generally associated with motor, sensory, cerebellar and cranial nerve dysfunction. Isolated abducens nerve palsy is rare [1, 2] and due to a small lesion which affects the radicular fascicles of the 6th cranial nerve and spares the abducens nucleus [3] . Conversely, abducens nucleus lesions produce a conjugate paralysis [4] but not an isolated lateral rectus muscle palsy. We report a case of unilateral conjugate gaze paralysis due to a small pontine haemorrhage involving the abducens nucleus which turned into a paralysis of the lateral rectus muscle.

One-and-a-half syndrome in ischaemic locked-in state: a clinico-pathological study.

Five patients with "locked-in" syndrome and dysconjugate palsy of horizontal gaze were studied. In all cases internuclear ophthalmoplegia due to dysfunction or destruction of the median longitudinal fasciculus was combined with an ipsilateral gaze palsy, producing the "one-and-a-half" syndrome. Clinical and electro-oculographic examination suggested involvement of the paramedian pontine reticul...

Macrocephaly in association with pontocerebellar hypoplasia type 1: a paradox.

The present case describes a patient with one and a half syndrome due to pontine granuloma (neurocysticercosis). In one and a half syndrome, the lesion affects the paramedian pontine reticular formation (PPRF) (or the abducens nucleus) and the medial longitudinal fasciculus (MLF) on the same side (the MLF having crossed from the opposite side). It is most often caused by multiple sclerosis, bra...

A Case Report of WEBINO Syndrome with Convergence Impairment

A 73-year-old man suddenly presented with alternating exotropia, bilateral medial longitudinal fasciculus (MLF) syndrome, and impaired convergence. This symptom has been known as wall-eyed bilateral internuclear ophthalmoplegia (WEBINO syndrome). Diffusion-weighted images on magnetic resonance imaging showed a smalllocalized lesion in his pontine tegmentum. Based on he had some ischemic risk fa...

Narcolepsy Associated with Duane’s Syndrome

The best characterised disorder of REM sleep, narcolepsy has never previously been associated with Duane's syndrome, in which there is developmental failure of the abducens nerve and its nucleus. The major brain stem nuclei responsible for REM sleep generation are situated in the pons in close proximity to the abducens nerve nucleus. We report the novel combination of Duane's syndrome and narco...

Congenital crocodile tears: a key to the aetiology of Duane's syndrome.

The occurrence from birth of copious lacrimation on eating in some patients with Duane's syndrome suggests that both are caused by dysgenesis or a lesion in the vicinity of the abducens nucleus in the pons.
